How to make a claim
PetFirst is committed to reimbursing policyholders quickly and work hard to minimize the time it takes to process a claim, but there are a few things you can do to ensure your claim is processed as quickly as possible.
When you take your dog or cat to the vet — whether for a routine treatment or an unexpected emergency — preparation is important to ensure your claim is processed quickly and you are reimbursed as soon as possible. It takes 10 business days to process a claim after receiving all the necessary information, and not having that information can delay the processing of your claim.
There are 3 items necessary to file a claim:
- Claim form (view PDF)
- Itemized receipt
- Written medical records (from the previous year if you are filing your first claim, otherwise incident specific)
You can login to MyPets to get a personalized claim form and track the progress of your claim from receipt to the issuing of your reimbursement check. The following steps will help expedite your claim's processing:
- Take your claim form with you to the vet.
Keep a few copies of your personalized claim form in your glove compartment or in your purse, so you have it when you visit the vet. Your claim form must be signed by your veterinarian, or PetFirst will not be able to process your claim. - Ask the receptionist to make a copy of your pet's written medical records at checkout.
The written medical records pertaining to the accident or illness treatment you are claiming are necessary to process your claim. PetFirst also requires that policyholders submit their pet's medical records for the previous year when filing their first claim. This information helps the claim adjudicators understand your pet's medical history. The better they understand your pet's health over the past year, the faster they can process your claim. - Submit these three items by mail:
- Claims form
- Itemized receipt
- Written medical record
- Forward your claim to:

PetFirst will not be able to process your claim until all these items are received. To make this process even faster, you can have your vet fax the information to 877-281-3348. The fax must be sent from your veterinarian's office. - Make a copy of all documents for your records.
Just in case your claim gets lost in the mail, it will be helpful to have a copy of everything to resubmit for reimbursement. - Track your claim's progress by logging in to your MyPets account.
MyPets will show you when your claim was received and when it was processed. You can also see if any information was missing from your claim as well as view the reimbursement amount.
